Introduction *
Diaphragmatic hernias encompass a spectrum of anatomical abnormalities involving a defect in the continuity of the diaphragm, allowing abdominal contents to pass into the thoracic cavity. They can be classified as congenital—resulting from embryological abnormalities during diaphragm formation, such as Bochdalek, Morgagni, or hiatal hernias—and acquired, generally secondary to chronic increased intra-abdominal pressure Bochdalek hernias account for up to 95% of congenital cases.
Material & Method *
A retrospective, observational, and analytical study was conducted at the Centenario Hospital Miguel Hidalgo in Aguascalientes, Mexico, including patients over 18 years of age diagnosed with diaphragmatic, hiatal, or parahiatal hernia, treated between January 2015 and August 2025. 185 cases were identified in the hospital electronic system: 37 met inclusion criteria. Variables Analyzed: Age, sex, habits, comorbidities, ASA classification, type of hernia, nature of surgery (urgent/elective), surgical technique (open/laparoscopic), operating time, bleeding, hospital stay, discharge and mortality.Statistical Analysis This was performed using SPSS v.22, applying Chi-square tests, Student's t-test, Spearman and Kendall Tau-b correlations, as well as multiple regression, considering significance with p < 0.05.
Results *
he results confirm the hemostatic advantage of the laparoscopic approach and its equivalence in clinical outcomes compared to open surgery. Despite the longer surgical time, laparoscopy offers superior visualization of anatomical structures, reduced blood loss, and faster recovery.
Conclusion *
The laparoscopic approach is a safe, effective option with less intraoperative bleeding , without increasing mortality or hospital stay.Multicenter studies with long-term follow-up are needed to standardize he indications for mesh use and the optimal repair technique.
