Introduction *
The Wagner and University of Texas (UT) wound classification systems are commonly used to assess diabetic foot ulcers and guide treatment. This study aimed to compare the frequency of high and low scores in both classification systems among diabetic foot patients and evaluate the associated risk of amputation.
Material & Method *
This cross-sectional study was conducted at Surgical Unit I, Lahore General Hospital, from 20th November 2023 to 19th May 2024. A total of 100 patients, aged 40–70 years and of both genders, diagnosed with diabetic foot were included. Patients with end-stage liver disease, coagulopathies, malignancies, or trauma were excluded. Each patient was assessed and classified using both the Wagner and UT systems. Follow-up was conducted over 8 weeks in the outpatient department. If the ulcer showed no healing or progressed, amputation was performed and recorded.
Results *
In our study, 26% of patients had high Wagner scores, while 74% had low scores. For the UT classification, 13% had high scores and 87% had low scores. The frequency of amputation was 34.62% in patients with high Wagner scores compared to 13.51% in those with low scores. Similarly, 53.85% of patients with high UT scores underwent amputation, compared to 13.79% with low scores.
Conclusion *
Both classification systems help stratify risk in diabetic foot patients. However, a higher proportion of amputations occurred in patients with high UT scores compared to high Wagner scores, suggesting the UT system may be more sensitive in predicting poor outcomes.
