Sarcopenia can lead to adverse outcomes such as physical disability, poor quality of life and mortality. Aim of the study - To determine the role of sarcopenia on postoperative outcomes in an emergency setting of bowel obstruction and its association with the degree of postoperative complications (Clavien-Dindo).
This study from August 2024 to May 2025 was done with a total sample size of 55 based upon statistical calculations. All patients receiving operative management treatment for Acute Bowel Obstruction were included and those receiving preoperative radio chemotherapy were excluded. Based upon the preoperative CT Abdomen based Psoas Muscle Index considering the cut off values as 6.36 cm2/m2 for men and 3.92 cm2/m2 for women in Asian population, the study population were divided in two groups: Sarcopenia and Non-Sarcopenia and evaluated for their surgical outcomes.
95.7% experienced postoperative complications in the sarcopenia group in comparison to 87.5% amongst non-sarcopenia. Based upon the Clavien-Dindo classification, the prevalence of moderate to severe complications was significantly higher (Grade 2-3A: 76.6%) in the sarcopenia. In contrast, the non-sarcopenia these complications overwhelmingly fell into the mild category (Grade 1: 75.0%, Grade 2: 12.5%) with no higher-grade complications. Remarkably, all the severe complications (Grades 3, 3a, 3A) and the single mortality, occurred solely within the sarcopenia. Logistic regression analysis demonstrated that sarcopenia remained a significant predictor for adverse outcomes (p<0.001), establishing a perfect model.
Sarcopenia index determination by Psoas Muscle Index calculations can be perfect model to predict adverse outcomes in laparotomies for gastrointestinal emergencies.
