Colorectal carcinoma is among the most prevalent malignancies globally, commonly metastasizing to the liver and lungs. Metastasis to the head and neck region, particularly the parotid gland, is exceptionally rare and undocumented in existing literature
We present the case of a 32 year-old male diagnosed with rectal adenocarcinoma who, following neoadjuvant chemo-radiation and surgical resection, developed isolated metastases to unusual head and neck sites, including the parotid gland, masseter muscle, and tongue. Histopathological and immunohistochemical analysis confirmed the rectal origin of these lesions. Despite systemic chemotherapy, the disease progressed, and palliative care was eventually initiated.
This case underscores the aggressive and unpredictable nature of rectal cancer metastases, highlighting the diagnostic challenges of atypical secondary lesions in the head and neck. It reinforces the importance of a multidisciplinary approach, thorough histological evaluation, and the role of otolaryngologists in managing rare glandular metastases in advanced disease.
Metastasis of rectal adenocarcinoma to the parotid region is extremely rare and signifies advanced disease with poor prognosis. This case broadens our understanding of metastatic patterns in colorectal cancer and emphasizes vigilance in evaluating unusual presentations.
