Introduction *
La reparación quirúrgica de las hernias epigástricas/incisionales asociadas con la diastasis de rectos es un tema controvertido, ya que una mejor reconstrucción de la línea alba es un procedimiento estético menos efectivo, una ubicación profunda de la malla requiere una disección más amplia y compleja con mas riesgo de complicaciones, se propone una técnica original: Reparación Trans Umbilical Sublay de Linea Alba (TUSLAR)
Material & Method *
Experiencia inicial en 18 pacientes con 3-7 años de seguimiento con hernias de la linea media asociada a Diastasis de rectos con una modificación de eMiLOS transumbilical, abordaje retromuscular asistido con espaciador de balón y cierre de linea media/apertura retromuscular con engrapadora Echelon cefalica y caudal con reforzamiento de sutura continua y malla retromuscular, con monitorizacion laparoscopica lateral de 5mm
Results *
Sin recurrencia en un seguimiento de 4 años (3-5 años) no evidencia de infección, recurrencia, dolor o abultamiento
Conclusion *
La TUSLAR es efectiva con excelentes resultados estéticos, pero se requieren mayores estudios a largo plazo
