La hernia inguinoescrotal es uno de los defectos más voluminosos dentro del espectro de hernias inguinales y representa un reto técnico debido a la longitud del saco y la distorsión anatómica. El abordaje laparoscópico permite una visión amplia del espacio preperitoneal y una reparación anatómica controlada. El objetivo de este video es mostrar nuestra técnica paso a paso y los resultados en un paciente operado en nuestro centro.
Se presenta el caso de un paciente masculino, R.J.H., con hernia inguinoescrotal izquierda reducible. Se realizó plastia inguinal izquierda más orquiectomia izquierda por vía laparoscópica TAPP bajo anestesia general. Se utilizaron tres trocares estándar. Se efectuó apertura amplia del peritoneo, disección completa del saco inguinoescrotal descendido hacia el escroto y exposición del espacio preperitoneal. Se colocó una malla de polipropileno 3D Max extra grande izquierda de 12.4 × 17.3 cm, con adecuada adaptación anatómica, seguida del cierre hermético del peritoneo con sutura continua.
El tiempo quirúrgico fue de 200 minutos, debido a la disección cuidadosa requerida por el gran volumen del saco herniario. No se presentaron complicaciones intraoperatorias. El paciente mostró adecuada evolución posoperatoria, reincorporándose de forma satisfactoria. A los 07 y 12 días de seguimiento, no se observó seroma, dolor significativo ni datos de recidiva.”
La reparación laparoscópica TAPP con malla 3D Max es una opción segura y eficaz para hernias inguinoescrotales de gran tamaño cuando se realiza bajo una técnica sistematizada. La disección cuidadosa del saco y la correcta restitución del espacio preperitoneal permiten resultados tempranos satisfactorios y sin complicaciones.
