Russia-Ukraine war shows various kinds of ballistic injuries due application of all kinds of weapon against Ukrainian soldiers. These injuries are frequently severe and complicated, with frequent diagnosis of acute kidney failure. The aim of this study was to investigate and evaluate utility of hemodialysis in combat patients treated in deployed filed hospital in Ukraine.
A 222 combat patients were diagnosed with acute kidney failure after ballistic injury and treated with 536 sessions of hemodialysis. Hemodialysis was performed in Role 4 deployed field hospital.
A continuous renal replacement rherapy (CRRT) was performed in 123 (55.4%) patients requiring 211 hemodialysis sessions, whereas persistent hemodialysis was provided to 99 (44.6%) patients with hemodialysis sessions 325 (60,6%). There were following indications for the hemodialysis: 63 (28.4%) patients with gunshot wound to the extremities with prolonged compartment syndrome, 59 (26.6%) patients with combined gunshot wounds; 47 (21.2%) patients with gunshot-related sepsis, 12 (5.4%) patients with burns; –17 (7.6%) patients due to exposure to chemical weapon; 22 (9.9%) patients with pancreatonecrosis, pneumonia, liver cirrhosis. The higher frequency of bleeding was detected in 30 (24%) patients with CRRT as compared to 10 (10%) patients with persistent hemodialysis (p=0.0079).
Hemodialysis might be in use in deployed filed hospital. Hemodialysis is useful tool to treat combat-related kidney failure with a relatively low rate of bleeding.
