Introduction *
This study aimed to evaluate and compare the effectiveness and safety of Hem-o-Lok® clips versus Endo-loops for appendiceal stump closure in patients undergoing laparoscopic appendectomy at a tertiary care hospital in Eastern Uttar Pradesh region of India.
Material & Method *
A randomized controlled trial was conducted involving 30 patients, equally divided into two groups: 15 patients received Hem-o-Lok® clips and 15 received Endo-loops. Patient demographics (age, sex, BMI, comorbidities) were recorded. Key outcomes assessed included total operative time and duration of postoperative hospital stay. Secondary outcomes included intra-operative blood loss, surgical site infection, stump leaks, intraperitoneal collections, bowel obstruction, and other morbidities within 40 days post-surgery.
Results *
The Hem-o-Lok group demonstrated a shorter mean operative time (46.7 minutes) compared to the Endo-loop group (55.2 minutes), indicating a time saving of approximately 8.5 minutes. The average length of hospital stay was slightly reduced in the Hem-o-Lok group (4.73 days) compared to the Endo-loop group (5.07 days). No complications such as infections, stump leaks, collections, or bowel obstructions were observed in either group. Intraoperative bleeding was minimal (<20 mL) in all cases, and no patients required postoperative drain placement. Baseline demographics were comparable between groups, with a mean age of 29.8 years and a slight female predominance (53%).
Conclusion *
Hem-o-Lok® clips are as safe and effective as Endo-loops for appendiceal stump closure and offer modest advantages, including reduced operative time and a shorter hospital stay. These findings suggest Hem-o-Lok may be preferable for routine laparoscopic appendectomies, though further large-scale studies are recommended to validate these results.
