Introduction *
Non-Hodgkin lymphoma (NHL) is a heterogeneous group of malignant neoplasms derived from lymphoid cells. Its presentation in the salivary glands (SG) is uncommon, accounting for approximately 5% of all tumors in this location. NHL most commonly develops in the parotid gland, representing around 75% of reported cases. The incidence of NHL has increased in recent decades. Risk factors include immunosuppression, viral infections such as Epstein-Barr virus (EBV), and autoimmune disorders. Diagnosing NHL in the SG is challenging since initial symptoms are often nonspecific, such as the presence of a painless mass or changes in saliva production. Imaging studies, including ultrasound, computed tomography (CT), and magnetic resonance imaging (MRI), are useful tools for evaluating the location and extent of the disease. However, definitive diagnosis generally requires fine-needle aspiration (FNA) or biopsy. The aim of this paper is to present a clinical case with the diagnostic and therapeutic algorithm of a patient diagnosed with NHL in the SG.
Material & Method *
The patient’s medical records and database entries were reviewed.
Results *
Imaging studies: solid, heterogeneous lesion in the left parotid gland measuring 45 x 40 mm. Fine needle aspiration (FNA): medium and large lymphoid cells. Immunohistochemistry (IHC) of incisional biopsy: Diffuse large B-cell NHL. Treatment: R-CHOP regimen (rituximab, cyclophosphamide, doxorubicin hydrochloride, vincristine sulfate, and prednisone)
Conclusion *
NHL should be considered in the differential diagnosis of salivary gland tumors. Cytology may have diagnostic limitations that should be complemented with IHC methods. The R-CHOP regimen is the treatment of choice.
