Abstract
Oral or Poster
Elective surgery for gallbladder stones is usually easy; however, when the indication is acute cholecystitis, with or without stones, the procedure duration, techniques and results are often unpredictable. A new method will be presented that allows a safe cholecystectomy to avoid damage to the common bile duct (CBD).
This new technique consists of identifying the cystic duct at the infundibulum between the inner wall of the gallbladder and the inflamed outer than the cystic duct can be clipped. It is fundamental to have a robust inner layer, because the mucosa can be thin, and the clip positioned on the cystic duct can be released, causing complications.
From January 2019 to December 2023, 4 patients (2 males and 2 females) underwent this new technique without complications on CBD.
This technique is a new possibility in cases of very difficult acute cholecystitis, but it is very important to identify the space between the inner wall of the gallbladder and the inflamed outer wall. When this is not possible, the traditional procedure is necessary for the benefit of both patients and surgeons.
