Introduction *
Aim. To investigate the effectiveness of the treatment of the intrabdominal testis using staged laparoscopic traction orchiopexy by Shehata technique
Material & Method *
Between 2022 and 2025, 12 boys aged 1 to 16 years underwent surgery. One patient underwent video-assisted one-stage orchiopexy and one underwent two-stage orchiopexy; the second stage was performed through inguinal canal. Ten patients underwent Shehata technique – the average age was 14 months. One patient was diagnosed with bilateral cryptorchidism.
Results *
The duration of the first stage lasted 32,5 min. After visualizing the testicle, we immediately sutured it behind the parenchyma with braided suture Ethibond 2/0, and performed traction, thereby preserving two working hands for a gentler dissection of the spermatic cord elements, completing fixation to the contralateral anterior superior iliac spine. The second stage was performed exactly after 8 weeks. The average duration of the operation was 45,0 min. In all cases, orchiopexy was performed by passing the testicle through the medial inguinal fossa (Prentiss maneuver), thereby creating a direct path and gaining a minimum 2-3 cm of spermatic cord length. We did not observe any intraoperative or postoperative complications and testicular atrophy.
Conclusion *
Staged laparoscopic traction orchiopexy by Shehata technique is technically simple, safe, and fast in the first and second stages. According to our data, performing the second stage after 8 weeks (2 months) and applying the Prentiss maneuver allows for sufficient vascular elongation and performing orchiopexy in the scrotum without tension. This approach can be recommended as a day-case procedure producing an ideal cosmetic result without complications.
