Our objective was to analyze the histopathological results of thyroidectomies performed during a 15-year period.
Data of all thyroidectomy patients from 1/1/2005 until 31/12/2019 were prospectively collected.
A total of 5725 cases were included (female: 76%, mean age: 48.6±8.7 years). Most common final diagnosis was multinodular goiter (n=2920, 51%) and malignant neoplasm including all histological types (n=1775, 31%). Papillary thyroid carcinoma (PTC) composed the vast majority of malignancies (n=1598, 27.9% of the total group and 90% of the malignant cases) whereas follicular cancer was less often identified (1.5% of the total and 5% of malignancies) as well as medullary carcinoma (1.4% of the total and 4.4% of malignancies). Main PTC histological variants were classic papillary (56.3% of PTC cases) and follicular (34.4%) while more aggressive subtypes were reported in 9.3%. Mean PTC diameter was 2±0,5cm (range: 0.1-7cm) with microcarcinoma representing 55.4% of these cases and 15.5% of the total group. Surprisingly, high percentages of capsule infiltration (50% of PTC and 14% of the total population), multifocality (55% of PTC and 15.4% of total) and bilateral carcinomas (40% of PTC and 11.2% of the total group) were observed in PTC patients.
Our data from a Greek population showed that malignancy was diagnosed in approximately one third of thyroidectomies. Although microcarcinoma represented half of PTC cases, aggressive subtypes were found in a relatively significant portion of them and, additionally, thyroid capsule infiltration as well as multifocality were noticed in half of these patients while bilateral carcinomas were identified in 40% of them.
