Traumatic intercostal herniation is a very rare entity with only case reports in the literature. We present the case of a gentleman who suffered a minor fall with undiagnosed rib fracture then developed intercostal herniation two weeks later, presenting in distress.
Computed tomographic imaging performed revealed the presence of anterior costal cartilage fractures with the lung herniating out into the chest wall deep to the pectoralis muscles. He was electively intubated for symptom relief (as he was in respiratory distress) then underwent a chest wall reconstruction with a biologic mesh followed by titanium plates to restore bony stability. Images from the scans and post operative pictures/videos will be included in the poster/presentation.
He went on to have an uneventful recovery and was discharged after a week. He had complete resolution of symptoms on follow up in the outpatient clinic and was back to his premorbid function within a month.
While rare, timely detection and treatment with a multidisciplinary team allows for excellent outcomes in intercostal lung herniation.
