Introduction *
Above knee amputation (AKA) comprises 37% of all limb amputations. Serotonin 5-Hydroxytryptamine (5-HT) and beta endorphin plays crucial role in pain management, stress reduction and mood regulation. The immediate postoperative prosthesis (IPOP) has been purported to allow early mobilization with potential physical and psychological benefits to patients. The current study compares the impact of IPOP with conventional prosthesis and assesses its effect on stress blood markers and quality of life.
Material & Method *
A pilot randomized controlled trial in patients with traumatic AKA, with a sample size of 60 is conducted at level I trauma center. The patients are randomly divided into 2 groups; Group A intervention IPOP group and group B conventional prosthesis group. Patients of group A are given IPOP within 24 hours of amputation along with definitive prosthesis later, while patients of group B are given conventional prosthesis when the stump matures. Outcome is assessed at baseline, 6 weeks, 12 weeks and 6 months for stress biomarkers (Serum beta endorphin and Serotonin 5-HT), pain (McGill Pain Questionnaire) and psychological well-being (WHO BREF QoL and Hospital anxiety depression score).
Results *
Forty patients are enrolled in the study (group A n=21 and group B n=19). Interim analysis is done to check the trend of intervention, along with any side effects of interventions are assessed. No adverse effect is reported by the patients.
Conclusion *
IPOP seems to be beneficial in early rehabilitation of traumatic AKA patients as compared to conventional prosthesis.
