Appendiceal lymphomas are very rare tumors. The incidence of primary lymphoma of the appendix is about 0.015% of all gastrointestinal lymphoma cases. This study aimed to report our experience in the management of three different cases of appendiceal non-Hodgkin lymphoma neoplasm with different presentations, histological findings, and immunoreactivity.
Here, we report a case series of three cases of appendiceal lymphoma who underwent surgical resection from 2012 to 2022. The ages were 3, 53, and 75. Two cases were presented with ileocecal masses on CT scans, whereas one case was presented with ileocecal intussusception. Ileocecal resection was done in all three cases.
Histopathological examination of the resected specimen showed non-Hodgkin lymphoma, and immunohistochemistry showed diffuse large B-cell lymphoma, germinal center type, diffuse large B-cell lymphoma, anaplastic variant, and extranodal marginal zone lymphoma, respectively. One patient completed six cycles of postoperative chemotherapy CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) and is under close follow-up. While the other two patients are receiving chemotherapy.
These reported cases demonstrate the importance of histologically examining appendectomy specimens. Clinicians must maintain a high index of suspicion for appendiceal neoplasm when encountering unusual findings of the appendix in a CT scan or during an operation. All appendectomy patients must undergo a histopathological examination.
