Compared to multiport laparoscopic surgery (CLS), laparo-endoscopic single-site surgery (LESS) has significant advantages in treating early-stage endometrial cancer (EC), such as reduced scarring. Nevertheless, the integrative research analyzing the surgical and oncologic outcomes is lacking. Our objective is to assess the clinical outcomes of postoperative complications and long-term results of LESS and CLS in early-stage endometrial cancer (EC).
A comprehensive electronic search on PubMed (Medline), Cochrane Central , Web of Science and other targeted searches were conducted from inception to March 2025 . A total of 17 studies (2 RCTs, 15 cohort studies, n = 2,659 patients) were selected while adhering to PRISMA’s guidelines for LESS and CLS comparisons for stage I-II EC. The primary outcomes for this research were the hospital stay along with operative time and estimated blood loss (EBL). Additional outcomes included complications and several clinical parameters such as lymph node yield, cancer recurrence, overall survival, along with patient satisfaction. Risk of bias was evaluated using RoB 2.0 and Newcastle-Ottawa tools. Random-effects meta-analyses were performed.
The subgroup and sensitivity analyses were undertaken to strengthen the validity of the findings. LESS significantly decreased blood loss and length of stay while improving satisfaction and lymphadenectomy rates. No substantial variability was noted in other analysed outcomes. Heterogeneity varied across outcomes, otherwise, all other results had either low to moderate heterogeneity.
Less appears to be safe for early-stage EC, it reduces blood loss and hospital stay, improves satisfaction, and depends on surgical expertise as compared to CLS
