Introduction *
Telomerase reverse transcriptase (TERT) promoter mutation is associated with poor prognosis in papillary thyroid carcinoma. The aim of this study is to compare the clinicopathologic characteristics and recurrence-free survival according to TERT promoter mutation status.
Material & Method *
We analyzed the medical records of patients who underwent thyroidectomy between January 2019 and December 2022 at Seoul St. Mary's Hospital. Out of a total of 2,593 patients, TERT promoter mutation was confirmed in 60 patients (2.3%).
Results *
Before propensity score matching (PSM), the mutated TERT group showed older age, larger tumor size, and more aggressive features (higher frequency of vascular invasion, advanced T and N stage). Notably, in the group under 50 years of age, there were no differences in clinicopathologic characteristics. In contrast, the group over 50 years of age showed additional differences in clinicopathologic characteristics (such as sex and lymphatic invasion). However, after PSM, there were no significant differences in clinicopathologic characteristics and recurrence rate based on TERT mutation status. Kaplan-Meier analysis revealed no significant difference in recurrence-free survival between the two groups, both before and after PSM.
Conclusion *
Patients with TERT promoter mutations exhibited more aggressive features compared to those with wild-type mutations, particularly in those aged 50 years or older. There were no differences in recurrence rates between patients with and without TERT promoter mutations. Due to the short-term follow-up period, the impact of TERT mutations on recurrence cannot be accurately determined. Further long-term follow-up studies are needed.
