Recurrent phyllodes-tumor(RPT) surgeries are challenging, can affect aesthetics & self-confidence. Obtaining apt surgical margins is goal, but preserving cosmesis is important. It is more relevant in resource-constrained settings, where dedicated breast surgeons are seldom found,surgical advances scarcely utilized. We aim to assess impact of oncoplastic breast conserving surgery(BCS )for RPT in preserving cosmesis, using BREAST-Q tool.
27 patients with RPT, April/2021-December/2024. Appropriate BCS technique employed, according to tumor site, size. 3 scales of Breast-Conservation Therapy module of BREAST-Q(version-2.0)used,namely: Psychosocial well-being(PSWB),Physical well-being:Chest(PWBC) and Satisfaction with breasts(SWB). For mastectomy, same scales of Mastectomy module used.
20 patients-benign PT, 4 borderline, 3 malignant. Mean-age 27.18+/- 6.30 years. 8 patients had tumor in upper-outer quandrant, 6 lower-outer, 2 central, 3 upper-inner, 2 lower-inner and 6 involved multiple-quadrants. Round-block mastopexy done in 12, transposition flap reconstruction in 5, superior pedicle-based resection & inferior-fold plasty in 2. Raquet mammoplasty & Grisotti technique was done in 1. 4 underwent mastectomy. When post-op means compared,difference in improvement significant in OPT-group (p-value of 0.002,0.005 and 0.001 respectively)
Utilization of BCS provide meaningful cosmetic results in recurrent PT, with remarkable patient satisfaction.
