Introduction *
A 'bezoar' is a mass of undigested material found in the gastrointestinal tract. Constant hair ingestion (trichophagia) can cause compaction of the mucosa in the gastrointestinal tract, forming a 'trichobezoar', which may trigger an obstruction requiring emergency surgery.
Material & Method *
This descriptive observational study examined a patient at the Mexican Social Security Institute – Regional General Hospital 01 in Sinaloa. Data from the patient's medical history was collected to prepare this report.
Results *
A 51-year-old female patient with a history of hyperprolactinemia, arrives at the emergency department with acute abdominal pain. There are no signs of peritoneal irritation or palpable visceromegaly. A CT scan of the chest and abdomen was performed, revealing clear evidence of intestinal obstruction and complete occupation of the gastric volume, with findings suggestive of a bezoar. In light of the above findings, the patient underwent emergency supraumbilical laparotomy, revealing total occlusion of the small intestine 90 cm from the fixed loop. An enterotomy was performed to remove a 9x5 cm trichobezoar. Based on data from the CT scan, a gastrostomy was subsequently performed on the anterior gastric wall to obtain a trichobezoar in the form of the entire stomach, which was removed
Conclusion *
We present a complex clinical case involving a patient with an intestinal obstruction caused by an extremely rare condition. Rapunzel syndrome is a complex condition that can accompany numerous other diseases. However, the relationship between hyperprolactinemia and bezoar formation is an area that requires more rigorous investigation, as it could set a new precedent in future research.
