Introduction *
Background. Chronic Venous leg ulcer (CVLU) is a significant global health challenge, predominantly resulting from chronic venous insufficiency (CVI), requiring effective treatment to enhance healing and quality of life. multilayered compressive bandages/ stocking is mainstay treatment of CVLU but takes long time to heal. This study is aimed to evaluate the efficacy of Hyperbaric Oxygen Therapy (HBOT) in conjunction with four-layer compression bandage (4LCB) therapy in VLU management.
Material & Method *
Adult Patients having CVLU due to primary varicose veins were included in the study . chronic venous insufficiency was diagnosed by duplex Scan and incompetent superficial veins and perforators were treated by laser. Patients were randomized by computer generated numbers and allocated in group A (n=35) and Group B (n=35). In group A patients got HBOT along with 4 LCB and in Group B patients received only 4LCB ,Adjunctive treatments like flavonoids and pentoxifylline were given in both groups .Followed for 8 weeks and reduction in ulcer size measured. QoL was measured by charing cross venous ulcer questionnaire and EQ-5D-5L.
Results *
Group A showed a significantly greater reduction in ulcer size compared to Group B (p=0.006). Quality of life scores improved significantly in both groups, with Group A demonstrating better Charing Cross scores (p=0.033). A moderate positive correlation was found between healing time and pre-therapy ulcer size in both groups.
Conclusion *
The addition of HBOT to standard four-layer compression bandage therapy significantly enhances ulcer size reduction and quality of life in patients with chronic venous ulcers.
