Intra-thoracic sleeve migration is an underreported complication of laparoscopic sleeve gastrectomy (LSG), its occurrence is presented by many symptoms like epigastric pain, persistent vomiting, and gastroesophageal reflux disease (GERD) .
All the patients enrolled in this study completed at least one year after LSG. Gastro-oesophageal symptoms not responding to medical treatment were assessed through esophagogastroduodenoscopy. Confirmed diagnosis were offered posterior cruroplasty and Roux-en-Y gastric bypass (RYGB).
Three hundred LSG cases were included in the study. Incidence of intra-thoracic sleeve migration was found to be 17%. The most common presentation was persistent vomiting (76.5%) , then refractory GERD (56.9%). Regarding management, all cases agreed to undergo surgical revision (posterior cruroplasty and RYGB).
Intra-thoracic sleeve migration should be considered in LSG patients presenting with vomiting and/or GERD. Posterior cruroplasty and RYGB is a valid option if not the best for repair even after successful weight-loss LSG.
