Introduction *
Penetrating injuries to the subclavian artery are usually the result of gunshot or stab wounds. While injuries to the subclavian artery are relatively rare, vascular injuries from penetrating trauma are common in trauma centers. Despite advances in modern medicine, these injuries are associated with high mortality and can result in devastating morbidity. We report the case of an elderly man with a pseudo-aneurysm of the right subclavian artery.
Material & Method *
This case was followed up sequentially by history taking, physical examination, Computed Tomography Angiography (CTA), and open surgical management, with follow-up status recorded during hospital stay.
Results *
In the operating theatre, the patient was placed in the supine position under general anesthesia and a linear incision was made in the right subclavian region. The skin and fascia were opened artery and vein identified and the wound explored. There was a ruptured arteriovenous branch and a ruptured capsule of pseudo-aneurysm, incision was made and evacuation of about 1000 cc of thrombus and ligation of arterial branch to control bleeding, skin was closed layer by layer and treated with open wound management using tulle and gauze. Post-operative status was stable and the patient was discharged from the hospital.
Conclusion *
The patient's condition demonstrated optimal arterial and venous blood flow, with minimal postoperative complications.
