Introduction *
Incidence of chronic venous leg ulcer (CVLU) is 1-2%., Caused by Chronic venous insufficiency.Four layer compressive bandages (4LCB) along with treatment of incompetent superficial veins and perforators is mainstay treatment of CVLU but it takes longer time. This study is to compare the efficacy of autologous Platelet-Rich Fibrin(PRF) versus normal saline dressings in Chronic CVLUs, focusing on ulcer area reduction and quality of life.
Material & Method *
All patients underwent Duplex venous USG . Only primary Varicose vein having ulcer were included in the study.A RCT was conducted on 74 patients with CVLUs Patients were randomized into two groups: Group A(n=36) received PRF dressings with standard 4LCB, while Group B (n=38)received normal saline dressings with 4LCB from toes to knee. (Dressings changed on a weekly basis for 4 consecutive weeks). Ulcer size and quality of life (CCVUQ and EQ-5D-5L scores) were assessed over 4 weeks
Results *
: Group A demonstrated a statistically significant reduction in ulcer area (17.46 to 9.92 cm²; p = 0.007) over 4 weeks, while Group B's reduction (14.07 to 11.45 cm²) was not significant (p = 0.913). CCVUQ scores improved significantly in the PRF group (63.64 to 59.22; p < 0.001), indicating better ulcer-specific quality of life. EQ-5D-5L scores improved in both groups but did not show statistical significance.
Conclusion *
Autologous PRF is a safe and effective adjunct to standard compression therapy in managing CVLUs, leading to superior wound healing and improved ulcer-related quality of life over normal saline dressing.
