La cirugía bariátrica guiada por fluorescencia representa una herramienta emergente que mejora la seguridad quirúrgica, permitiendo la identificación temprana de posibles complicaciones intraoperatorias.
Se realizó un análisis retrospectivo de 200 procedimientos bariátricos asistidos por fluorescencia: 130 mangas gástricas y 70 procedimientos de bypass (incluyendo bypass gástrico en Y de Roux y otros tipos). Se evaluó la utilidad de la fluorescencia en la identificación de complicaciones inmediatas.
La fluorescencia con ICG permitió detectar áreas de mal perfusión tisular, sangrados activos y posibles fugas en anastomosis durante el procedimiento. En varios casos, estas observaciones permitieron una intervención oportuna que previno complicaciones mayores en el postoperatorio inmediato.
El uso de verde de indocianina en cirugía bariátrica, en especial en cirugía de revisión, ofrece ventajas significativas al permitir la visualización en tiempo real de la perfusión tisular y la detección temprana de complicaciones críticas. Su implementación puede considerarse una herramienta complementaria para aumentar la seguridad en procedimientos bariátricos complejos.
