Introduction *
Primary hyperparthyroidism in child bearing age is 8 new cases per 100000 women per year(1). Based on Indian PHPT registry prevalence in pregnancy is 2.1%(2) while western population reports less than 1%(3). Diagnosis during pregnancy and postpartum becomes challenging as symptoms of the disease are masqueraded by physiologic non- specific pregnancy symptoms delaying treatment. Awareness of the presentation during this period is crucial as it is a preventable cause of morbidity and mortality in the mother and the child.
Material & Method *
The demographic details, clinical presentation, obstetric history, mode of delivery, neonatal history and treatment were retrieved from inpatient admission in nine along with details from Electronic medical records (EMR) in one from August 2018 to August 2024.
Results *
Ten women with median age of 24.5 years (range 21-38 years), 80% were less than 30 years of age. Interestingly all the patients had symptoms related to hypercalcemia during pregnancy even though eight presented and were diagnosed after delivery. Bone presentation predominated in 90% followed by renal calculi, all treated surgically and cured, two in pregnancy, parathyroid adenoma was the cause in 90%.
Conclusion *
This is a study from a tertiary referral center in South India. The study was conducted to increase the current knowledge of PHPT presentation and management in pregnancy and postpartum. Bone was affected in the majority (90%). Amongst the new-borns 50% had low birth weight and one child required calcium supplementation up to two years of life. Surgery is the curative treatment and most common cause is a parathyroid adenoma.
