Solid pseudopapillary tumor (SPT) of the pancreas is a rare epithelial neoplasm with low malignant potential, predominantly affecting young women. Although its clinical presentation is often nonspecific, complete surgical resection offers high cure rates.
We report a case structured according to CARE (CAse REport) guidelines of a 17-year-old female with an SPT located in the pancreatic tail. The case was prospectively documented, integrating clinical data, imaging studies, operative and histopathological findings, and long-term follow-up.
The patient presented with abdominal pain, postprandial fullness, and weight loss. Contrast-enhanced computed tomography revealed a solid mass measuring 16 × 14 × 12 cm in the pancreatic body and tail, without evidence of metastatic disease (Figure 1). A distal pancreatectomy with splenic preservation was performed, using vascular sealing technology and a mechanical stapler to optimize hemostasis and reduce the risk of pancreatic fistula. Histopathological examination confirmed a well-encapsulated SPT with no vascular or perineural invasion, negative surgical margins, and a disease-free regional lymph node. Postoperative recovery was uneventful, with hospital discharge on the seventh day (Figure 2, Figure 3). Over a five-year period of clinical and imaging follow-up, the patient remained free of recurrence or metastasis, maintaining good quality of life (Table 1).
Pancreatic SPT should be considered in the differential diagnosis of pancreatic masses in young women. Complete resection with splenic preservation, when feasible, is the treatment of choice and is associated with excellent survival rates and low morbidity.
