The choice between mastectomy and breast-conserving surgery (BCS) for non-metastatic breast cancer significantly impacts long-term quality of life (QoL). While oncologically equivalent, their effects on psychological, sexual and physical well-being remain critical to personalized care.
This cross-sectional study compares QoL outcomes in 210 non-metastatic breast cancer survivors (enrolled: 150; target: 210; 92 mastectomy, 58 BCS) using the validated BREAST-Q questionnaire. Domains assessed include satisfaction with breasts, psychological well-being, sexual well-being, and physical function. Non-parametric tests (Mann-Whitney U) compared outcomes between surgical groups, with significance at p < 0.05.
Interim analysis (n = 150) revealed significantly higher satisfaction with breasts in the BCS group (82.4 ± 18.7 vs. 70.1 ± 21.3, p = 0.008) and superior sexual well-being (88.6 ± 17.2 vs. 81.3 ± 20.1, p = 0.023). No significant differences were observed in psychological well-being (98.5 ± 3.2 vs. 97.8 ± 5.1, p = 0.412) or physical function (85.4 ± 14.6 vs. 83.9 ± 16.8, p = 0.672). However, mastectomy patients reported higher rates of chronic pain (24% vs. 12%, p = 0.038) and body image concerns (31% vs. 18%, p = 0.045).
: BCS is associated with better breast satisfaction, sexual well-being, and fewer body image concerns compared to mastectomy, though both groups report similar psychological and physical function outcomes. Chronic pain remains a notable issue post-mastectomy. Upon completion (N = 210), this study will strengthen evidence for surgical decision-making, emphasizing the integration of patient-reported outcomes into clinical guidelines to optimize long-term QoL.
