Introduction *
Congenital midgut malrotation is a rare anatomical anomaly that may cause duodenal or small bowel obstruction, and is seldom diagnosed beyond infancy. Symptomatic cases may present with acute intestinal obstruction or ischemia due to midgut or cecal volvulus, or with chronic, nonspecific abdominal pain.
Material & Method *
A case report from our surgery departament.
Results *
We report the case of a 26-year-old woman who presented to the emergency department with signs of small bowel obstruction. Abdominal CT demonstrated a “whirlpool sign”, indicating intestinal malrotation with midgut volvulus. The patient underwent a Ladd procedure, involving derotation of the bowel to a nonrotated position, preservation of bowel viability, division of Ladd’s bands, and widening of the mesenteric base.
Conclusion *
Small bowel obstruction due to volvulus complicating malrotation is an uncommon presentation in adults and can be effectively treated with the Ladd procedure.
